Main duties will be completing Care Act assessments to determine eligibility for care, reviews, support plans, funding panel request, and covering duty in the absence of the duty worker.
- Knowledge of literacy and numeracy skills, to write factual reports to meet the needs of Agencies/ Departments, to maintain appropriate records including financial assessments and to assist people with their finances. To produce high quality assessments
- An appreciation and understanding of current issues in social work practice.
- Ability to use IT systems to input and access data, use word processing systems and Emails.
- Understanding of and commitment to the Councils Equality and Diversity policy.
Assessing the needs of elderly people and people with disability.
Working with multiple agencies to facilitate holistic assessments. To collaboratively support people in achieving their goals, outcomes and maximisation of their independence.
Identifying safeguarding and quality of care issues. To ensure that client's welfare is protected and that the quality and standard of support provided is optimised.
